application for mail-in ballot each calendar year; repeals automatic 
vote-by-mail designation.

Enacted by the Senate and General Assembly of 
the State of New Jersey:

���� 1.��� Section 16 of P.L.1974, 
c.30 (C.19:31-6.4) is amended to read as follows:

���� 16.� a.� The Secretary of 
State shall cause to be prepared and shall provide to each county commissioner 
of registration forms of size and weight suitable for mailing, which shall 
require the information required by R.S.19:31-3 in substantially the following form:
